    Overview of service

The support at Conquest House is delivered by dedicated and professional multi-disciplinary team committed to providing quality care focused on the needs of the individual. We believe in a personalised approach and everyone living at Conquest House has their own Person Centred Plan which is reviewed regularly. Our aim is to provide the highest quality support enabling individuals to build on skills, live more independently and reach their goals.

Individual choice is paramount and they are actively encouraged to make informed choices and maintain independence. A key worker is assigned to each resident ensuring a holistic approach to individual care.

We encourage individuals and families to actively participate in the planning of care, which is facilitated through individual/relatives meeting where information is provided. We work closely with local services such as the community learning disability team, community nurses, psychologists, speech therapists, local health centres, dental, chiropody and other health care services.

What it's like to live here

Our activities co-ordinator organises a wide range of daily activities both in and out of the service which are planned in conjunction with the wishes of the people living there. These include: swimming, horse riding, bowling, trips to cinema, outings to pubs and shopping.

College courses are available and include music, art, communication, speech, cooking, relaxation, arts and crafts, dancing or gardening.

Our own transport is used for visits to local places of interest and to go out to local day services and resource centres.

People are encouraged to pursue and retain their own interests. One-to-one support is provided where required and all activities are arranged to meet their individual needs.

Everyone has access to the kitchen and can make themselves teas, coffees or cold drinks. They are also encouraged to keep their rooms tidy and do their own personal washing.
